A portion of health and dental premiums are covered based on the number of hours a position is budgeted. Please note the following for enrollment:
- Employees enrolling in a Family Health and/or Dental Plan need to submit a copy of their Marriage Certificate if adding a spouse
- If adding a Domestic Partner, a Domestic Partnership Affidavit must be provided. The IRS requires you to pay taxes on the value of your domestic partner’s health and dental benefits
- Birth Certificates for each dependent child
- Children can be on your health and dental plan up to age 26

Health Insurance
Employees may choose from two plans:
- CHA Option (HMO) – requires that all covered members have a CHA Primary Care Provider (PCP) who manages their care and they receive all their care at CHA
- CHA Choice Plan – a tiered plan that allows employees flexibility as to where they receive services
- Tier I offers care at CHA or BIDMC and its clinical affiliates with little or no co-pays
- Tier II includes in-network provider care and requires a deductible and co-pays or co-insurance
- Tier III allows for out of network provider care with a significant deductible and co-pay
Both plans are administered by Health Plans Inc. a wholly owned subsidiary of Harvard Pilgrim Health Care
Dental Insurance
- Dental coverage offered through Delta Dental
- Calendar year maximum per member/dependent is $1,500
- As a full time employee, 100% of your dental premium is covered
- As a part-time employee, a percentage of the premium is paid based on number of hours worked
Malpractice Coverage
- Coverage provided through Risk Management Foundation (CRICO)
- $5/$10 Million in coverage
- Modified Claims-Made Coverage
Professional Expense Reimbursement (CME)
- Money provided for Continuing Medical Education Credits
- Provided on a fiscal year basis (7/1 – 6/30)
- Pro-rated by FTE and start date
Retirement Plan - 403 (b)
- A percentage of your pay up to the Federal Limits contributed on a pre-tax basis
- Transamerica Retirement Solutions is the plan record keeper
- Match up to 2% of employee’s pre-tax contribution (1/3% for each 1% up to 6% employee contribution)
- A Discretionary Deposit of up to 2% is voted on annually by PO Board
- Full vesting after 2 years of service with 1000 hours completed each of the two calendar years
- Catch-up provision available for employees age 50 and over (these funds not matched)
Short-Term Disability
- Coverage provided for short-term disabilities from the 7th day through the 90th day of a medically verifiable illness (including medical portion of Maternity Leave)
- Benefit paid at 70% percent of base weekly salary up to $3,000 weekly. Paid on a tax-free basis
Long-Term Disability
- Coverage is provided for disabilities longer than 90 days and up to age 65
- The total amount of coverage is sixty percent of base monthly salary, with a maximum of $15,000 monthly. Paid on a tax-free basis
Life & Accidental Death Insurance
- A basic life plan benefit of one and one half times (1.5) your annual salary up to $300,000 for both life insurance and Accidental Death and Dismemberment (AD&D)
- Additional life or AD&D coverage may be purchased at the employee’s expense
- Coverage over the guaranteed issue amount requires evidence of insurability
Flexible Spending Accounts
- Employees may contribute to Flexible Spending Accounts to cover certain expenses with pre-tax dollars
- Health Care Spending Account – Used to set aside monies to cover out-of-pocket expenses not covered by medical or dental insurance plans
- Dependent Care Spending Account – Used to set aside monies to cover expenses for child care for a dependent child under the age of 13 which allows you and/or a spouse to work
Care.com Backup Child Care
Emergency day care benefit provided through Parents-in-a-Pinch
- Long-term dependent care assistance available via Child Care Resource Center referral service
